This article explains how to configure the different question types in the performance forms, and shows how to use them when writing feedback.
Note only Administrators and employees with edit rights for Performance & Development settings can create and configure performance feedback forms.
What are performance feedback forms?
Performance feedback forms are templates that you can configure under Performance & Development > Settings. You can use them when providing employee feedback during performance review cycles or when giving continuous feedback. With these templates, you can customize the information to collect regarding an employee's performance.

Configure the questions
After creating a form, you have the option of adding different types of questions and configuring the type of answer you would like. For each question, you can:
- Add a general description.
- Set the question as mandatory.
- Enable comments (except for open text questions).
Mark a question as private
You can mark any question in the form as private. This means that the question and its answer are not visible to the receiver of the feedback. This also applies when exporting the review to the employee's documents tab.
Private questions are still visible to the reviewer, and to those who have access to the review:
- Administrators and employees with edit rights for cycles and cycle reviews.
- The supervisor of the feedback receiver.
- Any other supervisor along the reporting line, if the option is enabled.
To mark a question as private, click the three dots on the question, and select Private.
Note that a feedback form cannot contain only private questions. Also, self review and continuous feedback forms cannot contain private questions, so all questions show in the form.
Tips
▶︎ If a form section contains only one question and the question is private, then the whole section is private.

Question types
1.Open text
This type of answer allows you to ask open questions. The evaluator can then fill out their answer.
2.Binary
Binary questions offer two options. You can add a label for each option (for example, "Yes" and "No") and a description to each label.
3.Traffic light
This type of question offers three options: red, yellow, and green. You can add a label for each option, (for example, "Reached", "Partially reached"), and add descriptions to the labels.
4.Descriptive scale
These questions offer up to seven options, from negative to positive, that you can configure as needed (for example, "Needs improvement", "Meets expectations", "Exceeds expectations").
5.Numeric Scale
These questions offer a minimum of three and a maximum of eleven options (ranging from 0 to 10), allowing you to give rating answers. You can add labels to each end of the scale, such as "Outstanding" to "Critical" to indicate the range of ratings.
Tip
If you create a scale question to rate performance, consider setting up a performance rating question instead.
6.Multiple choice
These questions offer one or multiple options to select as answers. You can include a "add other" text field so that employees can record their answer when the available options do not apply.
Using performance feedback forms
After creation, the templates are available for selection when creating a performance review cycle, or when giving continuous feedback. Reviewers can then provide their answers in the selected form.
Personio indicates whether a question is mandatory or not, and a description appears when you hover the mouse over the respective label.
