Create feedback forms to use in performance review cycles, or when giving continuous feedback. You can configure the number and type of questions in the form.
Note Only Administrators and employees with edit rights for Performance & Development settings can create feedback forms.
To create a new form, navigate to Performance & Development > Settings > Forms templates. Click Create a new form.
The configuration page will open. To configure the template, follow these steps:

- To edit the form title, click the field at the top.
- Optional: Click the toggles to make the form available for continuous feedback, or to make it the default one.
- You can create sections to organize the template questions. To do so, click the Add a section icon
on the right hand side. Then click the relevant section to configure it.
- To add a question under a section, click the plus icon on the right hand side. Enter the name and the type of question. Configure the question as you wish.
- Optional: add a description for the question.
- Optional: To make a question mandatory, or to allow comments, click the respective toggles.
- Click Save form.
The new template is now available to use when writing feedback. Note that you cannot download empty templates yet. The functionality will be available in the future.
Tips
▶︎ To duplicate or delete sections and questions, click Duplicate or Delete
on the selected element.
▶︎ Use the handle icon to drag and drop questions where needed. To reorder a section, click the section name, then click the Reorder section icon
. Drag and drop the section as needed.
