This article explains how to configure the different question types in the Performance Form templates, and show how they are then displayed under the Performance tab.
What are Performance Form Templates?
Performance Form templates are templates that you can configure in your settings and which will be used for providing employee feedback in the Feedback section under the Performance tab in the employee profile. With these templates, you can customize the information to be collected regarding an employee's performance.
Configuring the Four Answer Types
After creating a form template, you have the option of adding new questions and configuring the type of answer you would like. You can combine several answers in one form.
1. Open Text
This type of answer allows you to ask open questions. The evaluator can then fill out their answer.
2. Binary Question
Binary questions allow you to configure two options between which the evaluator will be able to choose when filling in the form. You can add a label for each option, for example, "Yes" and "No". Descriptions can be optionally added to the label. You can also allow evaluators to add a comment to the option they choose and decide if this question is mandatory.
3. Traffic Light
This type of answer consists of three options: red, yellow, and green. You can add a label for each option, for example, "Reached", "Partially reached", "Not reached". Descriptions may also be added to the label if you wish. In addition, you can allow evaluators to add a comment to the option they choose and decide whether this question is mandatory.
4. Scale
Custom rating scales consist of a minimum of three and a maximum of five options, allowing you to offer 'rating' answers. You can add a label to each option, for example, a gradual rating from "Outstanding" to "Critical". Descriptions may be added to the labels if needed. You can also allow evaluators to add a comment to the option they choose and decide if this question is mandatory.
Using Performance Feedback Templates
The templates that you created in your settings will then be available for selection in the Feedback section under the Performance tab. Once you have selected the right template, the questions are displayed, and you can provide the answers in the format that was selected in the settings.
It is stated whether a question is mandatory or not, and a description will appear when you hover the mouse over the respective label.
