In this article, you will learn how to create and use standardized evaluation forms to collect clear and structured feedback in a consistent form from everybody involved in the recruiting process. Personio offers you the opportunity to use standardized evaluation forms. They are freely configurable and can be easily integrated into the application process.
Creating Evaluation Forms
Since there are different evaluation criteria for different interview types, you have the option of creating different evaluation forms. For example, when you first get to know each other on the phone, different criteria are important than in a technical interview with the future supervisor.
Create an evaluation form by navigating to Settings > Recruiting > Evaluations and using the input field Add new form... . Next, scroll down to the Form Sections to create all required sections, such as “Expertise”, to structure your evaluation sheet. Combine any number of elements that you want to query within a section:
|Heading||Headings allow questionnaires to be organized by different blocks.|
|Scale||You can quantify the evaluation of your employees using 5-point scales. If you use multiple scales, you have the option of depicting the relevance of individual questions by weighting with values between 0 and 1. On this basis, an overall result from all ratings is calculated.|
|Text field||Text fields also allow the input of qualitative feedback.|
Sections Key Take Aways and Overall Recommendation are by default added to each evaluation form to summarize evaluation.
To then assign the sections to the corresponding evaluation sheets, click on the respective evaluation sheet and add the section using the plus symbol.

Use of evaluation forms
Personio offers you three options for submitting evaluations:
- Via the tab Evaluation in the profile of an applicant: choose Create evaluation select an appropriate evaluation form, fill it in and save your evaluation.
- Via a feedback request: Click the button Request Review in the applicant profile, to request feedback from a colleague. Select evaluation form as review type and b determine the corresponding evaluation form . Decide whether and after how many working days the participant should be automatically reminded of the completion of the evaluation form by email. Once you send the request, the relevant employee receives an email notification, and an entry is made in their to-do list in the Personio dashboard.
Request feedback on an applicant using the type Review comment if you do not want to use a predefined evaluation form for this. You can add a description of any length to the request.
- Via an interview: When creating recruiting interviews in the application profile, you can select an evaluation sheet to be filled in by all participants after the interview. The participants are asked to submit an evaluation analogous to point 2.
If any fields in a questionnaire do not apply to an applicant, they can be left out from the evaluation. All evaluations are summarized and displayed chronologically in the overview of an applicant. You can also find them aggregated under the Evaluations tab.
The feedback giver is able to access open evaluations or review requests directly via their dashboard, the overview tab of the corresponding application profile or the Evaluations or Notes tab. As soon as the assessment has been submitted via one of those areas, the task is considered to be completed.
